Chucky's human form, Charles Lee Ray; is a man with white skin, blue eyes, hair on his shoulder. When it becomes a toy, it has red hair, freckles and blue eyes. She wears a colorful striped shirt, red shoes and a blue jumpsuit that says Good Guys. It looks like this in the first 3 films and in the 6th film. In the fourth film of the series (Chucky's Bride), his famous stitches and bloody eyes are revealed on his face. In the sixth series of the series (Chucky's Curse), it looks normal again. Because the stitch marks are covered with rubber. In the film, when Barb discovers the rubbers and removes them from Chucky's face, his scary face reveals again.
Chucky'nin insan formu Charles Lee Ray; beyaz tenli, saçları omzuna gelen, mavi gözlü bir adamdır. Oyuncak formuna girince kırmızı saçlara, çillere ve mavi gözlere sahip olur. O renkli çizgili gömlek, kırmızı ayakkabı ve üzerinde Good Guys yazan mavi bir tulum giyer. İlk 3 filmde ve 6. filmde bu şekilde görünür. Serinin 4. filminde (Chucky'nin Gelini) yüzünde onun meşhur dikiş izleri ve kanlı gözü ortaya çıkar. Serinin 6. filminde (Chucky'nin Laneti) yine normal görünür. Çünkü dikiş izlerini kauçukla örtmüştür. Filmde Barb kauçukları fark edip onları Chucky'nin yüzünden çıkarınca tekrar korkunç yüzü ortaya çıkar.
